Houses in astrology show us which area of life energies of planets play out in. Each house signifies a different area of life to pay attention to. For example, you might have Mars in the 4th house. PS - everyone has all 12 houses, but some may not have any planets in them.
You'll notice that the 1st house still starts in the east, but the east is now in alignment with the cardinal directions.
